In submitting this form I confirm that I have read and agree with the following reservation conditions:

Holiday cancellation: Should it be necessary for my holiday to be cancelled, I agree that in the first instance I will work with Levendis to reserve another holiday at a mutually agreeable time and within the next 12 months, in which case a 2% administration charge may be made. However, if this cannot be arranged, I agree that I forfeit the following percentage of the total cost of my holiday: cancellation prior to 60 days before holiday = 20%; 30 - 59 days before holiday = 40%; 15 - 29 days before holiday = 70%; and 0 - 14 days before holiday = 100%.

Damages: I agree to fully compensate Levendis Estate for any damages to its property caused by myself or the people named on this form.
